Licensed Vocational Nurse (LVN) Home Health

Health Link Home Health and Hospice, a renowned provider of comprehensive home health services in the greater San Francisco Bay Area for nearly 20 years, is seeking compassionate, organized, and team-oriented individuals to join our team as Full-Time, Part-Time, or Per Diem Home Health Licensed Vocational Nurses (LVN) in the East Bay. In this role, you will collaborate closely with our Director of Nursing, clinical staff, and front office team to ensure coordinated patient care and support our healthcare professionals.

Responsibilities

• Implement care plans initiated by the Registered Nurse
• Accurately and promptly document care activities
• Assess and educate patients and families about diagnoses and care plans
• Coordinate home health services and report needs to the Registered Nurse or Clinical Supervisor
• Use equipment and supplies efficiently
• Complete online charting in Kinnser timely
• Follow all company policies and procedures
• Perform additional duties as assigned

Pay Rate

$40-$50 per hour DOE

Requirements

• At least 1 year of nursing experience (home health or medical/surgical preferred)
• IV Certification and wound care/wound nursing experience
• Current LVN License in California
• Adherence to professional standards and practices
• Good verbal and written communication and organizational skills
• Current CA Driver's License
• Ability to travel within a 15-30 mile radius
• Coverage area: East Bay
