Job summary

Sweetwater Care in California seeks a licensed LVN to provide nursing care within the scope of practice, following physician orders and center policies. You will participate in quality improvement, educate patients and families, and support clinical coordination in a long-term care setting.

The role requires an active California LVN license, experience in long-term care, and familiarity with PointClickCare charting.

Qualifications

• California LVN license required or in process.
• Long-term care experience preferred.
• PointClickCare charting experience required.
• Nursing management experience preferred but not required.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in the Quality Improvement Program.
• Uses Quality Life Manual for patient identification, placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes and carries out physician orders when permitted by the Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
