Job summary

Sweetwater Care is seeking an LVN in California to provide nursing care within the scope of practice, following physician orders, center policies, and state and local guidelines.

The role requires familiarity with long-term care settings, medication administration, and charting systems such as PointClick Care. Knowledge of quality improvement and infection control is desirable, with opportunities to participate in rounds and family education.

Responsibilities

• Provides nursing care within scope of practice according to standards, physician orders, center policies and procedures, and local guidelines.
• Notifies departments of patient changes (rooms, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
