Job description

We are looking for experienced Licensed Vocational Nurses to work Continuous Care shifts.

We are looking for an LVN who possesses a sympathetic attitude toward caring for hospice patients and families and has positive communication skills interaction with other members of the hospice team.

Continuous Care shifts are frequently after hours and on weekends.

What you will do:

  1. Work under the direction of a Registered Nurse and provide nursing and palliative care to assigned hospice patients.
  2. Work assigned shifts of skilled nursing care.
  3. Perform prescribed medical treatments, including pain management and symptom control, and provide nursing observations.
  4. Provide teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary caregivers, and participate in the care plan development and implementation with other members of the hospice interdisciplinary team.
  5. Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
  6. Assist the RN in carrying out the plan of care.
  7. Perform other duties as assigned.

Skills, education, and experience:

  1. Graduate of accredited nursing program.
  2. Current, active, license as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
  3. Current CPR, TB, health clearance and clinical skills self-assessment.
  4. Successful passage (80%) of Agency competency exam and core competency skills checklist.
  5. Minimum of one year experience as an LVN.
  6. Knowledge of End-of-Life care and services.
