Qualifications
- Must have and maintain an active, unencumbered CA LVN license
- Current CPR certification (with hands-on component)
- TB skin test - 2 step (current within last 12 months)
- One year prior hands-on nursing experience
- Reliable transportation
- Proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
- Continuing Education as required by state

Position Overview
The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians' orders for each individual patient, while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions
- Responsible for the delivery and coordination of quality patient care in compliance with physician orders.
- Assess patient condition and care needs continuously and report changes appropriately.
- Document all activities, assessments, nursing actions, responses, and care coordination in a timely manner.
- Participate in, implement, and update the nursing care plan.
- Take appropriate nursing actions based on assessments to achieve expected outcomes.
- Recognize changes requiring intervention and implement care to prevent or reduce risks.
- Comply with policies, regulations, accrediting bodies, and the Nurse Practice Act.
- Provide care using infection control measures per OSHA standards.
- Educate patients and families on disease, self-care, and prevention strategies.
- Maintain knowledge through ongoing education, internal learning, and skill evaluations.
Requirements
- Graduate of an accredited nursing school.
- Current, unrestricted state license as a Licensed Nurse.
- Current CPR certification.
- Proficiency in clinical assessments, documentation, and compliance.
Additional State-Specific Requirements
- South Carolina – One year of pediatrics experience.
- California – One year of experience working under the current nursing license.
- Louisiana – One year of experience as a licensed nurse.
- Continued education as required by state.
Preferences
- Six months of recent clinical nursing experience.
- Home health experience.

- Attention to detail, time management, problem-solving, organization, and communication skills.
Physical Requirements
- Ability to speak, read, write, and understand English.
- Ability to travel and lift 50 pounds.
- Ability to reposition patients and move equipment without assistance.
- Prolonged standing, walking, bending, kneeling, reaching, twisting.
- Ability to respond physically and mentally to emergencies.
