NurseCore is currently looking for Licensed Vocational Nurse LVN in San Antonio and the surrounding area. Home Health nurses with Hospice experience and HomeCare/HomeBase software experience preferred.

Responsibilities:

Qualified nurses must possess critical thinking skills, decisive judgment, and the ability to work with minimal supervision in a fast-paced environment. One year experience in infusion nursing and home health experience is preferred.

• Provide quality in-home care and infusions as defined within the scope of practice by the Nurse Practice Act.
• Administer and teach self-administration of specialty pharmacy infusion therapies and injectables to patients with rare, chronic, and acute diseases in the home setting, infusion suites, or physician offices.
• Proficient in accessing and maintaining SPC/PIV/Midlines, CVAD (PICC/Implanted Ports), SCIG, phlebotomy, and lab processing. Follow proper infection control measures.
• Coordinate planning for the delivery of nursing care with the Director of Nursing and make appropriate clinical decisions in patient care. Proactively update patient care plans, health changes, and related incidents.
• Demonstrate knowledge of age-specific differences in the patient population, including physiological and developmental differences.
• Ensure safe patient care; adhere to policies, procedures, and standards, including time management, supply management, productivity, and quality of service.
• Willingness to travel within the assigned geographic region and flexibility to work alternate shifts on short notice; be on call for field visits as needed.
• Administer CPR and other emergency procedures as necessary.

Qualifications:

• Licensed as a Registered Nurse through the State Board of Nursing required.
• Home health experience preferred – capable of performing assessments, wound care, medication management, infusions.
• Minimum of two years of medical-surgical or critical care/ER experience preferred.
• Minimum of one-year recent infusion experience preferred.
• Proof of current PPD, current CPR (hands-on class), and COVID vaccinations.
• Successful completion of skills assessment specific to areas of experience.
