Job summary

STAT Home Health in Pearland, TX is seeking Licensed Vocational Nurses (LVN) for Full-Time and PRN positions. You will provide nursing and supportive care in patients' homes under RN supervision and coordinate with physicians and home health staff to ensure optimal care.

Ideal candidates hold a Texas LVN license with at least 2 years of LVN experience; home health experience is preferred. Dependable transportation, a valid driver’s license, and car insurance are required.

Qualifications

• Current Texas LVN license.
• Minimum of 2 years as a Licensed Vocational Nurse (LVN).
• Home Health experience preferred.
• Have dependable transportation, driver’s license, and car insurance.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrates knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
• Great time management skills
