Job summary

Paradigm Healthcare in Gahanna, OH is seeking a dedicated Licensed Vocational Nurse for a part-time night shift. You will provide direct nursing care under RN or physician supervision, following the plan of care and current orders, while upholding facility policies and resident privacy.

You will also lead and mentor nursing staff, perform rounds, and ensure adherence to nursing standards in a skilled nursing setting.

Qualifications

• Must possess a current, unencumbered active license to practice as a Licensed Vocational Nurse in Ohio.
• Minimum 2 years’ long-term care experience preferred.
• Six (6) months of experience in rehabilitative/restorative nursing preferred.
• Ability to demonstrate leadership, organizational skills, and a professional attitude.
• Strong familiarity with Point Click Care preferred.
• Computer literacy with Outlook, Microsoft Office and Internet.

Responsibilities

• Deliver care to residents following the plan of care and physician orders.
• Follow facility policies and supervise day-to-day nursing service functions.
• Maintain resident privacy and promptly report any HIPAA concerns or violations.

• Oversee nurse aides and coordinate with staff to uphold department policies.

Creekside Care Center of Gahannais a Skilled Nursing Facility that is seeking a dedicated and compassionate LVN to join our team!

Responsibilities

• Provide direct nursing care, under the direct supervision of a registered nurse or physician, in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgment.
• Deliver care to residents following their plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilities.
• Have knowledge of and utilize the facility’s written policies and procedures that govern the day-to-day functions of the nursing service department.
• Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Make frequent rounds of the assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
• Oversee nurse aides and communication with and oversight of nursing home staff to ensure that specific policies, procedures, and systems are maintained correctly as it relates to the department.

Qualifications & Requirements

• Must possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse in the State
• 2 years’ experience in Long-Term Care experience preferred.
• Six (6) months of experience in rehabilitative and restorative nursing practices preferred.
• Must be able to demonstrate leadership, organizational skills, and maintain a positive and professional attitude
• Strong utilization knowledge of Point Click Care preferred
• Computer literacy skills (Microsoft Outlook, Microsoft Office, Internet)
