Job summary

Sierra Health and Wellness in Orangevale, CA is looking for a Licensed Vocational Nurse to join our inpatient care team. The role focuses on providing compassionate, patient-centered nursing support under physician orders and care plans.

You will document medical records in EMR, monitor medications, and collaborate with psychiatrists, nurses, social workers, and therapists to support recovery and safety for our adult and family clients.

Qualifications

• Current LVN license in California.
• Ability to document in EMR systems.
• Strong communication with patients and families.

Responsibilities

• Oversee the day-to-day delivery of patient care.
• Provide outstanding, individualized resident care and medical support to clients as needed.
• Prepare and review medical records and nurses' notes to evaluate the resident's physical and emotional status.
• Provide care according to the Physician's orders, care plans, established standards, policies and procedures.
• Prepare and monitor patient administered medications as ordered by the physician in accordance with nursing standards and facility policies.
• Monitors and reports adverse reactions to medications and treatment to the RN and/or Physician.
• Sign, date and perform all charting, record keeping and care plans in accordance with established policies and procedures.
• Utilizes good infection control procedures.
• Utilizes behavioral management and crisis intervention techniques.
• Ensures compliance with State licensing standards and accreditation standards
• Adheres to and upholds the policies and procedures of the company.
• Other duties as assigned.
• Facilitate group therapy sessions.
• Provide client assistance in crisis intervention situations.
• Maintains EMR.
• Collaborate with other members of the healthcare team, including psychiatrists, psychologists, social workers, and occupational therapists.
