Job summary

Overview

This position is located within the Texas Health and Human Services Commission (HHSC) Lubbock State Supported Living Center, where the focus is on providing high‑quality direct services to individuals with intellectual and developmental disabilities who are medically fragile or have behavioral challenges. The LVN IV will deliver advanced vocational nursing care, supervise and train other LVNs, and collaborate closely with registered nurses and interdisciplinary team members to improve health outcomes.

Responsibilities

• Perform highly advanced vocational nursing work for residents with complex medical, mental health, and/or behavioral health issues.
• Observe, diagnose, and treat residents’ medical needs, including medication administration and specialized nursing procedures such as tracheostomy and gastrostomy care.
• Collect specimens for laboratory examination, conduct screening tests, and monitor vital signs and health status.
• Train, model, coach, and supervise other LVNs and direct support staff, providing instruction and feedback on nursing tasks and documentation.
• Enroll new LVNs in on‑the‑job training, complete training checklists, and provide mentorship throughout orientation.
• Assist registered nurses in implementing clinical protocols, quality improvement initiatives, and infection control procedures.
• Lead interdisciplinary collaboration, communicate pertinent information to team members, and provide input on nursing intervention outcomes.
• Respond to emergency situations and enforce safety and infection control procedures.
• Perform additional duties as assigned, such as assisting the infection control nurse, medication variance, and scheduling support.
• Participate in disaster response and continuity of operations activities as required.

Essential Job Functions

• Attend work on a regular basis, possibly on rotating schedules and overtime as agency policies allow.
• Perform technical nursing procedures in compliance with facility and state regulations.
• Administer medications and treatments by oral, rectal, internal, or injection routes.
• Make rounds and home visits as needed; work at alternate locations within the Lubbock SSLC when necessary.
• Monitor residents’ weight, diet, feedings, and vital signs following procedures, and report findings to RNs or primary care providers.
• Oversee monitoring of residents by direct support professionals after treatments or procedures.
• Document treatments and observations accurately in the electronic or paper medical record.
• Assist in quality assurance and therapeutic nursing program implementation.
• Respond to emergencies and provide safety training to staff.

Knowledge, Skills, and Abilities

• Knowledge of nursing care standards and procedures for patients with complex health needs.
• Knowledge of emergency procedures and equipment such as CPR.
• Knowledge of state and federal regulations governing Intermediate Care Facilities for individuals with intellectual disabilities.
• Ability to identify problems, evaluate solutions, and use reasoning to resolve situations.
• Computer proficiency in common software applications.
• Strong nursing care skills and capacity to train others.
• Organizational and evaluation skills for nursing activities and health service delivery.
• Effective oral and written communication skills, including policy interpretation.
• Capacity to provide care to individuals with complex medical, mental health, and behavioral health issues.
• Maintain accurate record keeping and use of electronic health records.
• Requirement for current CPR certification and physical ability to perform CPR.

Licensure and Certifications

• Licensed as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in Texas, or in a state that recognizes Texas reciprocity through the Nurse Licensure Compact.

Initial Screening Criteria

• Graduation from an LVN or RN program.
• 12 months of nursing experience (direct or volunteer work with individuals with intellectual or developmental disabilities is preferred).

Additional Information

• Experience in training, teaching, or leading others is preferred.
• Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including CANRS, Nurse Aide, Medication Aide, and HHS List of Excluded Individuals/Entities.
