Brief Job Description

This position is available for morning, evening, and night shifts across multiple homes. The LVN will perform advanced and basic nursing duties, including administration of medications, complex procedures such as tracheostomy and gastrostomy care, specimen collection, health screenings, patient monitoring, electronic health record documentation, training of direct support staff, and collaboration with the RN and interdisciplinary team. The role may qualify for additional pay for evening, night, and weekend work.

Essential Job Functions (EJFs)

Attends work regularly, may be required to work a rotating schedule, extended shifts, or overtime in accordance with agency leave policy, and performs additional duties as assigned.

May serve on call in a rotating schedule to meet operational needs.

Administers medications and treatments via various routes, conducts rounds and home visits, and operates under agency, SSLC, and state regulations.

Performs complex procedures such as tracheostomy care, gastrostomy/jejunostomy care, wound care, and breathing treatments; collects specimens for laboratory examination.

Monitors individuals' weight, diet, vital signs, and overall health status; responds to signs of illness; documents observations in the medical record.

Communicates pertinent information orally and in writing with team members; collects data for RN/primary care provider.

Leads, monitors, and oversees others’ monitoring work; evaluates intervention outcomes.

Trains/instructs new LVN staff, oversees their work, provides feedback, and supports the new LVN’s onboarding and ongoing development.

Assists RNs with implementation of protocols, quality assurance programs, and therapeutic nursing initiatives; participates in facility committees and quality improvement initiatives.

Responds to emergency situations, follows safety and infection control procedures, provides safety training, and performs other duties such as assisting infection control, medication variance, and scheduling.

Participates in disaster response and continuity of operations as required.

Knowledge, Skills and Abilities (KSAs)
  • Knowledge of nursing care standards, procedures, and techniques; understanding of principles for patient care.
  • Knowledge of emergency procedures, CPR, and ability to assess and care for residents or staff in emergencies.
  • Knowledge of state and federal laws, ICF‑IID regulations, policies, and procedures related to SSLC.
  • Skills in reasoning, problem identification, and resolution.
  • Computer operation skills and familiarity with applicable software.
  • Care and treatment skills for patients.
  • Ability to organize, coordinate, evaluate nursing activities, interpret policies and regulations, recognize medical necessity patterns, and plan and oversee work of others.
  • Ability to interpret regulations and communicate them effectively orally and in writing.
  • Ability to provide patient care to individuals with complex medical, mental health, and/or behavioral health issues.
  • Ability to prepare and maintain records using an electronic health record system.
  • Ability to communicate effectively and instruct others.
  • Must maintain current CPR certification; requires strength and flexibility for CPR.
Registrations, Licensure Requirements or Certifications

Licensed to practice as a Licensed Vocational Nurse in the state of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.

Initial Screening Criteria
  • Graduation from an LVN program.
  • 6 months of prior employment experience as an LVN.
  • At least 12 months of prior experience as an LVN is preferred.
  • Experience working or volunteering with persons with intellectual or developmental disabilities is preferred.
  • Experience in training/teaching or leading others is preferred.
  • Experience using an electronic health record system is preferred.

Flexibility in work hours may be required; the position may require overtime, extended hours, weekdays, weekends, and holidays.
