Adaptive Home Health, LLC is seeking a Licensed Practical Nurse (LVN/LPN) to deliver high-quality in-home care in Arlington, Texas. Full-time role with productivity incentives and per-point pay.

Must have Texas LPN license and 3+ years of nursing experience, CPR/BLS, and clean driving record. Responsibilities include wound care, medication administration, infusion therapy, patient advocacy, and collaboration with physicians and care teams.

Qualifications

Must hold current Texas LPN license and have 3+ years of clinical nursing experience.
Experience with home health care settings preferred.
Proficient with skilled documentation and Homecare Homebase.

Responsibilities

Provide comprehensive in-home nursing care.
Administer medications and treatments as prescribed.
Treat wounds and provide infusion therapy.
Coordinate care with physicians, referrals, and caregivers.
Participate in regular case conferences and trainings.
Manage about 6 patients per day, Mon-Fri.

Job description:

Seeking elite Licensed Practical Nurse! Previous home health experience preferred.

This position is full time with the expectation of 30 productivity points completed per week. The clinician will be paid per point and has the opportunity to earn additional pay if the clinician elects to perform more than 30 productivity points per week.

Position requirements:

LPN is responsible for providing comprehensive care in the patient's home, including the completion of skilled documentation through Homecare Homebase.

Comfortable treating wounds
Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
Administer medications and treatments as prescribed by physician. Provides direct patient care including infusion therapy, wound care, teaching, disease management.
Participate in regular case conference and in-services
Service ~ 6 patients a day M-F

Current Texas LPN license
