Job summary

Adaptive Home Health is seeking a Licensed Vocational Nurse (LVN) for field-based home health visits in the Houston area. The role is full-time, with Monday–Friday shifts and some weekend coverage as needed.

You will perform home nursing visits, conduct assessments, reconcile medications, and educate patients and families while coordinating with the RN/clinical team. Applicants should hold an unrestricted Texas LVN license, a valid TX driver’s license, and be comfortable with travel across the

Responsibilities

• Deliver skilled nursing visits in the home within LVN scope, under RN/clinical supervision.
• Perform focused assessments and escalate changes promptly to the RN/MD/clinical team.
• Support medication reconciliation and medication safety education within scope.
• Provide patient and caregiver education (teach-back) aligned to the plan of care.
• Support wound care and other skilled tasks per orders and competency.
• Coordinate care with the office and interdisciplinary team to ensure timely visits.
• Document thoroughly and on time.
• Work with operations to focus on patient care rather than logistics.

  • Breakdown: $44.40 per point
  • Mileage reimbursement
• Schedule: Full-time Monday–Friday + some weekends (as needed)

What You Will Do

• Support medication reconciliation and medication safety education (within scope), and identify discrepancies or risks for RN review.
• Provide patient and caregiver education (teach-back) aligned to the plan of care.
• Support wound care and other skilled tasks per orders and competency (e.g., dressing changes, basic monitoring, reinforcement of teaching).
• Coordinate care with the office and interdisciplinary team so visits happen on time and orders are followed.

Requirements

Must-haves
