Responsibilities

• Plans, provides, coordinates, documents and evaluates social services to patients.
• Includes group therapy (in person or virtual), case management, discharge planning, family meetings.
• Maintains admission, treatment and discharge documentation per hospital standards.
• Develops and updates Master Treatment Plan with clinical staff.
• Carries out treatments per the Master Treatment Plan for social services.
