Qualifications

• 3+ years of clinical experience in social work, mental health counseling, psychology, or a related field.
• Master's degree in Clinical Social Work, Mental Health Counseling, Psychology, or related field.
• Current Virginia state license as a mental health professional.
• Ability to work collaboratively within an integrated care team.

Responsibilities

• Assess patient status by obtaining health and mental health history through interviews and chart reviews, assess illness, risks, psychosocial factors, and culture.
• Coordinate with clinical leadership, physicians, and other clinicians as needed.
• Develop evidence-based and professionally respected treatment plans.
• Implement treatment interventions and monitor progress.
• Provide continuity of care and triage patient calls/ emails within 24-48 hours.
