Job summary

BayMark Health Services, Inc. is seeking a compassionate, full-time Licensed Substance Abuse Counselor to support patients in an outpatient setting in Kentucky.

The role focuses on counseling for opioid use disorder with trauma-informed practices and collaboration with a multidisciplinary team to plan individualized care. Qualifications

• Must hold at least one Kentucky license (LCADC, CADC, LCSW, CSW, LMFT, LMFTA, LPCC, or LPCA).
• Experience with chemical dependency populations preferred.
• Ability to conduct bio-psychosocial assessments.
• Demonstrates a positive, non-judgmental attitude.
• Clear background check and drug screening required.

Responsibilities

• Provide individual and group counseling services at prescribed intervals.
• Conduct initial assessments to determine service eligibility.
• Develop and review individualized treatment plans.
• Coordinate services with a multidisciplinary team.
• Address patient grievances and develop aftercare plans.
