Job summary

Old Dominion Rehabilitation and Nursing in Newport News, VA is seeking a Speech Language Pathologist (SLP) to provide direct patient care in a resident-centered setting.

You will assess patients, develop treatment plans under physician direction, document progress, and collaborate with the rehab team. VA licensure is required; prior long-term/geriatric care experience is preferred; you will join a team committed to quality care and staff development.

Qualifications

• Must have graduated from an accredited Speech Therapy program.
• Must have a valid VA license/registration.
• Experience in long-term or geriatric care is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to assess condition and rehab potential.
• Establish and administer a treatment program with physician direction.
• Set goals based on patient capacity.
• Document and record patient condition.
• Educate patients and families on therapy methods.
• Complete evaluations, progress notes, recertifications and discharge documents.
• Collaborate with all disciplines for patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Ensure confidentiality and HIPAA compliance.

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
  
  
  

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of VA
• Experience with long-term care and/or geriatric care is preferred but not limited to.
