Job summary

Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is seeking a licensed Speech Language Pathologist to provide comprehensive speech-language pathology services within an interdisciplinary team. The position requires an active or pending state SLP license; a Master’s degree in Speech Language Pathology is required and CCC-SLP certification is preferred or required.

CPR certification is preferred.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited program
• Active or pending state SLP license to apply for this position
• Current CPR certification preferred or required

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders through clinical assessments and interviews.
• Develop and implement individualized therapy plans based on assessment findings with measurable goals.
• Provide therapy to improve communication skills, including articulation, language, voice, fluency, and swallowing.
• Educate patients and their families about disorders, therapy techniques, and strategies to support progress.
• Maintain accurate documentation of evaluations, progress notes, and treatment plans in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
