Job summary

Quality Moments LLC is seeking a Senior Care Case Manager (LSW, LPC) to provide compassionate support and companionship to seniors in skilled nursing facilities. The role emphasizes dignity, personal connection, and fidelity to commitments while addressing social, emotional, and behavioral well-being.

The position may be filled as either part-time or full-time, with opportunities to engage elders through meaningful activities and individualized care plans.

Qualifications

• Must be at least 21 years of age.
• Diploma or GED required.
• Desire to improve quality of life for aging adults.
• Must have experience working with Alzheimer patients.
• High level of commitment, dependability, and interpersonal skills.
• Prior experience as an STNA, CNA, or in a nursing or long-term care setting is preferred.
• Prior experience in social work, counseling, human services, or a related field is preferred.
• Valid driver's license and reliable transportation with current automobile insurance meeting state minimum liability requirements.
• Previous EHR system use, and professional documentation experience are helpful.
