You will conduct psychosocial assessments, develop care plans, coordinate discharge, connect with community resources, and advocate for patient rights.

Qualifications

• Massachusetts LSW license required.
• Bachelor's or Master's in Social Work from an accredited program.
• Experience in skilled nursing/long-term care preferred.
• Excellent communication and interpersonal skills.
• Ability to handle sensitive situations with empathy.
• Pass background and employment checks.

Responsibilities

• Provide social work services to patients and residents per care plans.
• Conduct psychosocial assessments identifying needs.
• Develop and implement individualized social work interventions.
• Provide emotional support, counseling, and crisis intervention.
• Assist with adjustment to illness, disability, and long-term care.
• Advocate for patients' rights, dignity, and quality of life.
