Job summary
Healthcare Nursing Center is seeking a Licensed Social Worker (LSW) for a full-time position in Massachusetts. You will conduct comprehensive assessments, develop and evaluate care plans with a multidisciplinary team, and provide counseling to clients and families.

Responsibilities
- Conduct comprehensive assessments of clients' social, emotional, and psychological needs.
- Develop, implement, and evaluate care plans with a multidisciplinary team.
- Provide counseling and support to clients and families addressing social and emotional concerns.
- Liaise with community resources and external agencies to coordinate support services.
- Advocate for clients' rights within the facility and the broader community.
- Participate in team meetings and contribute to social work policies and procedures.
