Job summary

AristaCare Health Services in Plainfield, New Jersey, is seeking a Social Worker to plan, develop, organize, implement and evaluate social service programs at our facility. This is a full-time role with the in-house Talent Acquisition Team guiding the process.

The position requires an Associate Degree, licensure as LCSW or CSW, and 2–3 years of experience in skilled nursing or assisted living. We offer competitive rates and strong benefits through AristaCare.

Responsibilities

• Participate in community planning related to facility interests and resident/family needs.
• Engage in discharge planning, develop and implement social care plans and assessments.
• Involve residents and families in planning social service programs when possible.
• Assist in arranging transportation to other facilities when necessary.
• Refer residents and families to appropriate social service agencies when the facility cannot provide services.
