Loyola Medicine, a member of Trinity Health, is seeking a Social Worker to join the Care Management Team at Loyola University Medical Center in Chicago's western suburbs. The role involves psychosocial assessments and education as part of coordinated patient care delivery.

Responsibilities

• Provides psychosocial assessment, intervention, evaluation, consultation, and education to patients/families as part of care management.
• Identifies high-risk patients from psychosocial/financial perspectives and provides information, support, counseling, and referrals.
• Liaises for complex transitions/discharges between hospital and community agencies for exchange of information.
• Contributes to training of social work professionals within LUHS education programs.
• Coordinates placement and discharge planning with the interdisciplinary team and addresses barriers to safe discharge.

What you will do:

• Provides psychosocial assessment, intervention, evaluation, consultation, and education to patient/families as part of comprehensive care management services.,
• Identifies high risk patients from a psychosocial/financial perspective and provides information, support, counseling, care management and referrals to appropriate resources.
• Serves as a liaison for complex transitions/discharges between the hospital and community agencies for the exchange of clinical and referral information.
• Contributes to training of social work professionals as part of the education program at LUHS.
• Facilitates and coordinates details of placement and actual discharge of complex discharges to appropriate agencies. Discusses with interdisciplinary team the discharge plan and any barriers to a safe discharge.

What you will need:

• Master's degree required in Social Work.
• Licensed Social Worker (LSW) required; LCSW preferred.
• 1-2 years of previous job-related experience required.
• Experience in the ER or acute hospital setting preferred.
• Excellent communication skills.
• Ability to manage crisis situations calmly and effectively.
• Ability to work under stressful conditions and in difficult situations.

Compensation:

Salary Range: $30.00 - $46.51per hour

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
