Job summary

Qualifications

• Must be a Licensed Social Worker.
• Prefer experience in long term care and memory care.
• Must be able to work with a multidisciplinary team and have excellent written and verbal communication skills.
• HMO, Medicare and Insurance experience preferred.
• Knowledge of community resources and HIPAA regulations compliance.
