Job summary

Astera Health in Wadena, MN is seeking a full-time Social Worker to join the patient care team. This role focuses on discharge planning, coordinating with the care team, and connecting patients and families with community resources to meet medical, emotional, and social needs.

You will serve patients and families across all Astera Health settings, support self-determination, and respond to multiple demands while maintaining timely communication with providers and outside agencies.

Qualifications

• Bachelor’s degree in human services or related field such as Social Work, Psychology, Human Services, Counseling, or Sociology
• 2 years’ experience in human services related work in hospital, nursing home, or county human services agency
• Active license as a Licensed Social Worker.

• Assist with discharge planning addressing social, medical, economic, and emotional needs.
• Make referrals for long-term care, home care, and community-based services with follow-up.
• Know area resources and processes to obtain services for patients.
• Participate in Utilization Review and discharge planning process.
• Assist in planning/conducting programs for social problems and community health counseling.
• Assist in organization of support groups for patients and family members.
• Primary contact for swing bed referrals; coordinate multi-disciplinary meetings during hospital stay.
