Agape Hospice, a Tucson-area hospice, seeks a Medical Social Worker to implement standards of care and collaborate with the interdisciplinary team to support patients and families at the end of life.
The role requires a CSWE-accredited BSW, one year in health care with hospice experience preferred, and CPR certification. The employee may travel to patient residences and maintain required records.
The medical social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.
