• Experience in medical social work preferred.

Responsibilities

• Maintain confidential, comprehensive clinical case files.
• Conduct psychosocial assessments of youth referred to placement.
• Coordinate treatment planning and reviews for program participants.
• Provide direct life skills coaching.
• Provide crisis intervention and advocacy services.
• Case management and connect clients with community resources.
• Perform intakes and match new referrals.
• Other duties as assigned.
