Job summary

HonorHealth is seeking a Licensed Social Worker for Case Management at Shea Medical Center, Scottsdale, AZ. The role focuses on coordinating care, communicating with patients and families, and ensuring safe transitions across the health system.

The position requires an MSW with licensure and at least 1 year of experience as a social worker; a sign-on bonus of up to $5,000 is available along with a day shift, Monday–Friday schedule and weekend rotation as needed.

Qualifications

• Master's Degree in Social Work is required; licensure as a Social Worker is required or LMSW/LSW/LCSW depending on role.
• Minimum 1 year as a Licensed Social Worker or health field placement in MSW program required.
• Experience in case management preferred; licenses and certifications may be required by state.

Responsibilities

• Accountable for an assigned caseload with coordination across care teams to ensure smooth transitions.
• Collaborate with patients, caregivers, providers and external partners to coordinate care and referrals.
• Advocate for patients and utilize standards of care and guidelines to ensure quality outcomes.
• Support discharge planning, financial information communication, and post-acute planning as needed.
• Participate in development and maintenance of Case Management metrics and caseload management.
