Job summary

Douglas County is seeking a Licensed Social Worker to work one-on-one with individuals and families, addressing complex barriers to self-sufficiency through intensive, person-centered case management and goal planning.

You will connect clients to needed resources, help shape programs and workshops, collaborate with community partners, and advocate for best social services practices to achieve lasting, positive outcomes for a small rural community.

Qualifications

• Bachelor’s Degree in Social Work; AND two (2) years professional experience performing social work, case management; experience in interviewing clients, providing information and resources, and determining eligibility for various social service, medical or related programs.

Responsibilities

• Interview applicants to assess needs and eligibility for social services.
• Provide intensive case management and goal planning for individuals and families.
• Develop and maintain relationships with clients and community resources; create content and materials for outreach.
• Maintain case files, logs, and regular reports; ensure confidentiality.
