Job summary

Qualifications

Responsibilities

• Conduct comprehensive psychosocial and behavioral health assessments.

• Provide crisis intervention and supportive counseling when needed.
• Collaborate with physicians, nurses, behavioral health providers, and other care team members.
• Coordinate referrals and follow-up care.
• Maintain accurate and timely clinical documentation.
• Provide education and support to patients and their families.
