Job summary

Preferred Care Management is seeking a Social Services Director to lead the Social Services Department in a resident-focused setting in Potomac, MD. The role requires overseeing the development, implementation, and ongoing evaluation of social services to support residents in achieving their highest practicable well-being.

The director will complete or direct the comprehensive social services component of assessments and contribute to care plan goals and interventions tailored to each resident

Qualifications

• Experience in social services within a healthcare or residential setting.
• Knowledge of state and federal regulations governing social services.

Job description

The Social Services Director is responsible for overseeing the development, implementation, supervision and ongoing evaluation of the Social Services Department designed to meet and assist residents in attaining or maintaining their highest practicable well-being. This includes identifying the need for medically-related social services and ensuring that these services are provided in accordance with State and Federal regulations.

The Social Services Director will complete and/or direct/delegate the completion of the social services component of the comprehensive assessment. The Social Services Director will also contribute to and/or direct/delegate contribution of social services goals and approaches to the comprehensive care plan. These goals and interventions will be individualized to match the skills, abilities, and interests/preferences of each resident in compliance with Federal and State
