Job summary

Richmond Adult Day Care, part of Centers Health Care, seeks a Licensed Social Worker to support residents in Staten Island. The role focuses on assessing social and psychosocial needs and helping residents manage daily living.

You will develop individualized care plans, support admissions and transfers, and join interdisciplinary team meetings to ensure comprehensive care.

Qualifications

• Licensed Social Worker (LSW) or Master of Social Work (MSW).
• Previous Long-Term Care and/or Subacute experience required.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.

Responsibilities

Richmond Adult Day Care is actively hiring a Licensed Social Worker to work in our Adult Day Care located in Staten Island, NY.

DUTIES:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
