Job summary

Fundamental is seeking a Licensed Social Worker (LSW) to oversee social services within a facility, coordinating with discharge planners and families to schedule facility tours and follow-up procedures. Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards
• Develop and manage the initial contact with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures
• Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of the patient/resident
• Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure social service programs meet patient needs
• Keep abreast of current federal and state regulations as well as professional standards

The Licensed Social Worker LSW oversees the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards and works directly with discharge planners, family or responsible party to initiate tour of facility and follow up procedures.

Posted Salary Range

USD $28.85 - USD $35.10 /Hr.

Wage Commensuration

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
