Job summary

McGregor PACE is seeking a Social Worker to join an interdisciplinary team helping older adults remain safely in their homes while avoiding hospitalizations. As a PACE Social Worker, you will advocate for participants, conduct psychosocial assessments, develop care plans, visit homes, provide counseling, and connect families with resources.

The role emphasizes relationships, independence, and quality of life.

Qualifications

• Licensed Social Worker (LSW) or LISW licensed in Ohio.
• Master's Degree in Social Work.
• Valid driver's license and reliable transportation.
• Ability to travel throughout the service area for participant visits as needed.
• Excellent communication and documentation skills.

Responsibilities

• Conduct psychosocial assessments and ongoing participant evaluations
• Develop individualized care plans with the interdisciplinary team
• Complete home visits to assess safety, support systems, and barriers to independence
• Provide counseling, crisis intervention, and emotional support
• Connect participants and families with community resources
• Assist with advance care planning and care transitions
• Participate in interdisciplinary team meetings
• Support participants experiencing housing, caregiver, financial, behavioral health, or social challenges
• Coordinate with hospitals, nursing facilities, home care agencies, and community partners
• Ensure documentation meets CMS and organizational standards

Qualifications

Required:

• Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) in Ohio
• Master's Degree in Social Work
• Valid driver's license and reliable transportation

Preferred:

• Experience with older adults, geriatrics, Medicare/Medicaid, hospice, home care, behavioral health, or case management
• Knowledge of community resources and long‑term care services
