Job summary

IHA is seeking a Clinical Licensed Social Worker (LCSW) to provide behavioral health services to PACE participants in Philadelphia. The role conducts psychosocial assessments and delivers CBT, DBT, and psychodynamic therapy to support individuals facing trauma, anxiety, depression, and adjustment challenges.

You will work independently and within an interdisciplinary team, develop, implement, and monitor individualized care plans, and ensure confidential documentation in compliance with HIPAA.

Qualifications

• MSW required.
• Current LCSW license.
• Two years experience preferred with frail/elderly populations.
• Experience in multidisciplinary healthcare settings preferred.
• Knowledge of Medicare, Medicaid, and social service practices.
• Reliable transportation required.

Responsibilities

• Deliver individual and group counseling services

• Educate participants and families on aging, grief, dementia, and care resources

Position Overview

The Clinical Licensed Social Worker (LCSW) provides high-quality behavioral health and therapeutic services to participants in the PACE program. This role conducts comprehensive psychosocial assessments and delivers evidence-based interventions—including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and psychodynamic therapy to support individuals experiencing trauma, anxiety, depression, and adjustment challenges.

Working independently and as part of an interdisciplinary team (IDT), the LCSW develops, implements, and monitors individualized care plans. The role also provides crisis intervention, coordinates with physicians and care teams, and ensures accurate, confidential documentation in compliance with HIPAA and regulatory standards.
