Job summary

PruittHealth in North Carolina-Kinston is seeking a Social Services professional to assist in planning, implementing, and evaluating social services for residents, in alignment with care plans and directives from the Director of Social Services.

The position requires an LBSW and at least one year of supervised social work experience in healthcare, with strong communication and leadership skills to coordinate with other departments and community resources. Full-time, first shift, on-site.

Qualifications

• Must have an LBSW from an approved school of Social Work.
• Minimum of one year supervised social work experience in healthcare.
• Ability to make independent decisions when warranted.
• Ability to work tactfully with staff, residents, families and agencies.
• Demonstrates leadership and teamwork abilities.

Responsibilities

• Assist in planning, developing, organizing, implementing, and evaluating social services.
• Coordinate policies and procedures for the social services department.
• Identify social and emotional needs and develop care plans.
• Participate in discharge planning and resident assessments.
• Refer residents to appropriate community resources as needed.
• Maintain charts and progress notes describing resident responses.
