Cumberland Family Medical Center in Kentucky is seeking a licensed mental health clinician (LCSW/LPCC/LMFT) for a full-time, clinic-based position at the Hardin County Schools Healthy Kids Clinic. The role emphasizes age‑appropriate assessment and intervention, development of cost‑effective care plans, and multidisciplinary, patient‑centered quality of care across CFMC sites.

Required: current unrestricted license in the state, BLS for Health Care Providers, NPI prior to start date, and 1–5

Qualifications

One year of experience in mental health or related clinical settings.
Three to five years of experience in a related environment.
Master’s degree, licenses, certifications, or registrations as required.
Current unrestricted license in the state in which one practices, required upon hire.
Basic Life Support (BLS) for Health Care Providers.
National Provider Identifier (NPI) required prior to employment start date.

Responsibilities

Uses age-appropriate assessment and intervention skills.
Assists members in developing cost‑effective, appropriate health care plans through optimizing members’ resources.
Promotes multidisciplinary, patient-centered care considering personal, psychological, economic, and cultural needs.
Maintains knowledge of CFMC services/benefits, and relevant laws affecting health care planning.
Completes required documentation of services and plan of care per policy.
Participates in program development, orientation, and educational activities to empower members.
