Job summary

Cumberland Family Medical Center seeks a dedicated behavioral health professional (LCSW/LPCC/LMFT/LPCA) to provide comprehensive psychosocial assessments and care at our clinics. The role emphasizes counseling interventions, patient-centered, and culturally aware care, with collaboration across disciplines and strong resources linkage.

Requirements include a Master’s degree and licensure, 3–5 years in related settings, and a commitment to evidence-based practice.

Qualifications

One year of experience in mental health, hospital, medical outpatient clinic, FQHC, Home Health or long-term care.
Expertise with brief counseling interventions.
Demonstrated ability to provide leadership.
Advanced knowledge of local and national resources related to behavioral health care.

Responsibilities

Uses age-appropriate assessment and intervention skills.
Promotes multi-disciplinary care that is patient-centered and considers all aspects of members' personal, psychological, economic, and cultural needs.
Completes required documentation of services and plan of care per policy.
Participates in program development, orientation, and educational activities to enhance members' capacity to direct their own health care.
