Lake City Healthcare and Rehabilitation is seeking a Licensed Physical Therapist Assistant (PTA) to join our rehabilitation team in Lake City, FL. The PTA will collaborate with Physical Therapists to deliver high-quality therapy services that promote mobility, independence and improved quality of life for residents.

The ideal candidate has at least 1 year of experience in a skilled nursing or rehabilitation setting and a passion for helping residents achieve functional goals through personalized

Qualifications

• Current, unrestricted Florida PTA license.
• Minimum 1 year of experience in a nursing home, skilled nursing facility, or rehabilitation setting preferred.
• Strong communication, organizational, and teamwork skills.
• Commitment to providing compassionate and quality care to residents.

Responsibilities

• Assist Physical Therapists in providing safe, effective, and individualized therapy treatments.
• Follow established care plans, treatment protocols, and safety guidelines.
• Provide support during therapy sessions to help residents improve mobility, strength, balance, and overall function.
• Maintain accurate and timely documentation of therapy services provided.
• Communicate effectively with the therapy team, nursing staff, and other healthcare professionals.
• Maintain a clean, organized therapy area and ensure equipment is properly maintained.
• Identify and report equipment concerns, safety hazards, or maintenance needs promptly.
• Follow infection control policies and proper PPE procedures.
• Promote a positive, professional, and resident-focused environment.
