Responsibilities

Key Responsibilities

• Conduct thorough and ongoing assessments to identify the unique needs of each patient and craft personalized treatment plans.
• Communicate effectively with supervisors and a multidisciplinary team to report on individual patient progress and collaborate on care strategies.
• Develop and provide a schedule of appointments, home visits, and observations to ensure ongoing support for clients.
• Coordinate home visits and telehealth consultations, conducting in-depth chart reviews and staff interviews as necessary.
• Monitor and ensure the effective implementation of designated program plans, continually assessing their impact on patient health outcomes.
