Aveanna Healthcare is seeking an LPN to provide compassionate in-home nursing for adult patients in the Phoenix area. The role involves lifting assistance, transfers, medication administration, showering, and safety, all performed within an established care plan.

Qualifications

• Active LPN license in good standing.
• Current CPR certification (hands-on component).
• TB test within the last 12 months.
• Six months of hands-on nursing experience preferred.

Responsibilities

• Provide in-home LPN care for adult patients, including lifting assistance, transfers, meds, showering, and safety.
• Document patient care with Aveanna tablet and maintain accurate records.
• Communicate clearly with patients, families, and clinicians; follow care plans.
