Otterbein SeniorLife is seeking a PRN Hospice LPN to assist with on-call needs, continuous care and routine patient visits. You will provide bedside care and support families, working closely with the RN Case Manager to ensure compliance with Hospice Medicare Conditions of Participation and Ohio regulations.

Qualifications include graduation from an approved nursing program, an Ohio LPN license with CPR, and 2–4 years of hospice/home care experience.

Qualifications

• Graduate of approved nursing program.
• Valid Ohio LPN license with CPR.
• Minimum 2–4 years’ hospice or home care experience.
• Must be a licensed driver with a reliable automobile and insurance.

Responsibilities

• We are currently seeking an experienced and professional Hospice LPN.
• This role is responsible for providing assistance in caring for our Hospice patients.
• You will provide bedside care to patients, support families and caregivers and work closely with the RN Case Manager.
• Otterbein Hospice complies with Hospice Medicare Conditions of Participation, and Ohio Department of Health regulatory requirements.

Qualifications

• Education: Graduate of approved nursing program
• Licensure/Certification: Valid Ohio Licensed Practical Nurse (LPN) license, CPR
• Experience: A minimum of 2 – 4 years’ direct patient care experience in hospice or home care
• Since this is a community-based ministry, all LPNs are required to be licensed drivers with automobiles in good working condition and have automobile insurance in accordance with Ohio laws.
