GBMC HealthCare is seeking an LPN to provide direct patient care under RN supervision in a hospital setting. The role emphasizes nursing assessment, patient education, and timely documentation of care.
The position requires current Maryland LPN licensure, knowledge of nursing theory, and preferred 1 year of acute care experience. CPR may not be required for Hospice staff, and staff must maintain professional documentation.
Under direct supervision of the registered nurse provides direct patient care to assigned patients.
Knowledge of nursing theory and practice acquired through an accredited school of nursing.
One-year acute care experience preferred
Interpersonal skills, communication (verbal and written) are required in order to instruct and comfort patients and their families and maintain effective contacts with a variety of hospital personnel and maintain appropriate/adequate documentation.
Ability to perform and plan professional nursing care resolving complex patient care problems and dealing with patients and their families often during stressful situations.
Current registration with the Maryland State Board of Examiners of Nurses as a Licensed Practical Nurse.
Healthcare Provider (CPR Certification) – not required for Hospice employee.
Ability to stand and walk almost constantly (up to 90% of work time) and occasionally (up to 50% of work time) lifting and positioning patients.
Normal patient care environment with exposure to excessive noise, dust, and temperature.
Possibly exposed to adverse working conditions due to performance of patient care activities and exposure to communicable diseases.
Assists in gathering data to assess the physical, psychological, social, spiritual needs of patients.
Collaborates with the registered nurse and interdisciplinary/team members in planning for the needs of patients upon admission.
Reviews the care plans and provides input into updating and revising.
Supports in direct patient care to assigned patients utilizing clinical nursing skills and judgement.
Provides education and reinforcement of education for patients and family, and documents outcome in the medical record.
Initiates appropriate emergency measures when patient’s condition dictates need.
Prepares patients for tests and procedures.
Administers and documents medications according to policy and procedure.
Can appropriately delegate care to non-licensed personnel and report to the Registered nurse.
Report to the Registered nurse with a thorough concise report on patient condition changes or evaluation of interventions.
