Rochester General Hospital is seeking a Licensed Practical Nurse to support the Stroke Team on site in Rochester, NY. This 40-hour, Day/Evening position emphasizes education reinforcement, data review, follow-up support, and care navigation within the stroke care continuum.

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. The Licensed Practical Nurse is a key member of the care delivery team. The Licensed Practical Nurse communicates the needs of the patient and the team; collaborates with the multidisciplinary team, takes direction from the RN and delegates appropriately to the other members of the team. The Stroke Program Support position is 100% onsite at Rochester General Hospital and supports patients and families across the stroke care continuum. This autonomous, collaborative role focuses on education reinforcement, data review, follow-up support, and care navigation without a traditional patient assignment. The position offers flexible weekday hours with no weekends, holidays, or floating.
Graduate from an accredited School of Practical Nursing or equivalent required. Current licensure in State of New York as a Licensed Practical Nurse.
Strong communication, customer service, and interpersonal skills highly desired. At least 6 months LPN experience preferred.
Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.
