Job summary

The Evolving Solution Services is seeking a Licensed Practical Nurse (Spanish Speaking) in Worcester, MA to provide direct nursing support for GAFC/AFC programs under RN supervision, ensuring safe, person-centered care and compliance with state regulations.

Responsibilities include assessments, care plan implementation, in-home visits, medication oversight within scope, caregiver education, documentation, and coordination with an interdisciplinary team to support members and families.

Qualifications

• Active LPN license in Massachusetts (required).
• Graduate of an accredited practical nursing program.
• Minimum of 1 year nursing experience; home care, long-term care, or community health preferred.

Responsibilities

• Assist the RN with initial and ongoing clinical assessments of GAFC/AFC members.
• Support development/implementation of individualized care plans including ADLs.
• Conduct routine in-home/site visits to monitor member health and safety.
• Provide direct nursing care as appropriate, including basic treatments and health monitoring.
• Observe and report changes in member condition to the RN or healthcare provider.
• Educate caregivers and direct staff on basic health care tasks, infection control, and safety practices.
• Assist with medication oversight within scope of practice.
• Maintain accurate documentation including visit notes and incident reports.
• Coordinate with case managers, caregivers, and interdisciplinary team members.
• Attend bi-monthly team meetings for care coordination and case discussion.

Salary Range: $35.00 To $100.00 Annually

Job Summary:
The Licensed Practical Nurse (LPN) for the Group Adult Foster Care (GAFC) and Adult Foster Care (AFC) programs provides direct nursing support and assists in the coordination of care for members in community-based settings. Working under the supervision of a Registered Nurse (RN), the LPN helps ensure members receive safe, consistent, and person-centered care while maintaining compliance with all applicable state regulations.

Key Responsibilities:

• Assist the RN with initial and ongoing clinical assessments of GAFC/AFC members


• Support the development and implementation of individualized care plans, including Activities of Daily Living (ADLs)


• Conduct routine in-home or site-based visits to monitor member health and safety


• Provide direct nursing care as appropriate, including basic treatments and health monitoring


• Observe and report changes in member condition to the RN or appropriate healthcare provider


• Educate caregivers and direct care staff on basic health care tasks, infection control, and safety practices


• Assist with medication oversight and reinforce proper medication administration procedures (as permitted by scope of practice)


• Maintain accurate and timely documentation, including visit notes and incident reports


• Collaborate with case managers, caregivers, and interdisciplinary team members


• Attend and actively participate in bi-monthly team meetings for care coordination and case discussion


• Report suspected abuse, neglect, or exploitation in accordance with guidelines from theMassachusetts Executive Office of Elder Affairsand theDisabled Persons Protection Commission


• Respond to routine clinical concerns and elevate urgent issues to the RN as needed

Qualifications:

• Active Licensed Practical Nurse (LPN) license in the Commonwealth of Massachusetts (required)


• Graduate of an accredited practical nursing program


• Minimum of 1 year of nursing experience; home care, long‑term care, or community health preferred


• Experience with GAFC/AFC or similar community‑based programs preferred


• Valid Massachusetts driver’s license and reliable transportation required
