Job summary

CharterCARE Health Partners is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services in a home health setting. The ideal candidate will be responsible for administering care as per physician-approved plans, require current licensure, and possess experience in Medical Surgical nursing, preferably in community health. This role involves various physical duties and adherence to safety and infection control protocols.

Qualifications

  • Current LPN licensure in Rhode Island. Massachusetts state LPN licensure preferred.
  • BLS required within 60 days of hire. Active Driver’s license.
  • One to two years of Medical Surgical nursing in an acute care facility.

Responsibilities

  • Provide skilled nursing services in home environment as per plan of care.
  • Work includes lifting patients and assisting in therapy and treatments.
  • Must follow all safety and infection control policies.

Job description

Summary:Provide skilled nursing services in the home environment as directed in the plan of care developed by the Registered Nurse and Physician. The Plan of Care approved by the physician.

Licensure: Current LPN licensure in the State of Rhode Island. Massachusetts state LPN licensure preferred. BLS required within 60 days of hire. Active Driver’s license.

Education: Graduate of an accredited program for Licensed Practical Nurses.

Experience: One to two years of Medical Surgical nursing in an acute care facility. Prefer one to two years of community health experience in a certified home health care agency.

Working Conditions, Physical Environment and/or Safety Requirements: Requirement to lift 50 lbs. and assist a 200-250 lb. patient, using proper body mechanics. Must demonstrate knowledge of safety precautions. Involves intermittent sitting, standing, walking, and driving motor vehicle. Involves working with frequent interruptions and multiple telephone communications. Requirement to conduct self in tactful, diplomatic manner. Meets all safety requirement associated with Roger Williams Medical Center. Required to use eyes, hands, fingers with skill in administering therapy and treatments and completing all necessary paperwork. Must follow all Infection Control Policies and Universal Precautions. Regular Staff are required to work the hours for which hired including weekend and holiday coverage as established by the Home Care Department. Nurses must meet annual safety training requirements per hospital policy.
