Job summary

Interim HealthCare of Augusta seeks a compassionate Licensed Practical Nurse (LPN) to provide one-on-one, in-home care. This role provides an opportunity to make a meaningful impact on the lives of patients and their families.

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one, in-home care for clients in a private duty setting. If you are passionate about delivering personalized, high-quality nursing care while building meaningful relationships with patients and their families, we'd love to hear from you.

Responsibilities

• Provide skilled nursing care in the client's home according to the physician's plan of care.
• Administer medications and treatments as prescribed.
• Monitor and document patient condition, vital signs, and progress.
• Assist with activities of daily living as needed.
• Educate patients and family members on disease management and care plans.
• Communicate changes in patient condition to the supervising Registered Nurse and physician.
• Maintain accurate and timely documentation.
• Ensure a safe, clean, and comfortable environment for the client.

Qualifications

• Current, active LPN license in the state of employment.
• Current CPR/BLS certification.
• Previous home health or private duty nursing experience preferred but not required.
• Strong clinical assessment and communication skills.
• Compassionate, patient-focused, and dependable.
• Reliable transportation and valid driver's license (if travel is required).
