Job summary

Carolinaseniorcare is seeking a Licensed Practical Nurse (LPN) for a PRN, on-site role at Providence Place in High Point, NC. You will provide compassionate nursing care to residents, following physician orders and care plans, while collaborating with an interdisciplinary team to ensure safe, quality outcomes.

The ideal candidate is a graduate of an accredited Practical Nursing program, holds an active LPN license, and has at least 1 year of nursing experience with a preference for skilled

Qualifications

• Active LPN license in good standing.
• Current CPR certification.
• Minimum of 1 year nursing experience; skilled nursing facility preferred.

Responsibilities

• Deliver high-quality nursing care per physician orders and care plans.
• Administer medications and monitor resident responses.
• Observe and report changes to RN Supervisor and healthcare team.
• Maintain accurate clinical documentation in a timely manner.
• Support resident admissions, transfers, and discharges.
• Collaborate with nurses, therapists, and physicians to achieve outcomes.

What You'll Do

• Deliver high-quality nursing care in accordance with physician orders, care plans, and regulatory requirements.

• Observe, assess, and report changes in resident conditions to the RN Supervisor, Director of Nursing, and healthcare providers.
• Maintain accurate and timely clinical documentation.
• Support resident admissions, transfers, and discharges.
• Collaborate with nurses, caregivers, therapists, and physicians to ensure exceptional resident outcomes.
• Foster a positive, respectful, and compassionate environment for residents, families, and team members.
• Ensure compliance with all state, federal, and company policies and standards.

Qualifications

Education

• Graduate of an accredited School of Practical Nursing.

Experience

• Minimum of one (1) year of nursing experience required.
• Skilled nursing facility experience preferred.

Licensure & Certifications

• Current, active Licensed Practical Nurse (LPN) license.
• Current CPR certification.
