Job summary

Lehigh Valley Health Network (LVHN) invites applications for a Licensed Practical Nurse to join LVPG-S Family Medicine in Frackville. This full-time day shift role supports the physician practice with patient history, examinations, triage, and care coordination across the continuum.

The LPN will assist providers during procedures, manage tests and referrals, educate patients, and help maintain safety and quality in a fast-paced clinical setting.

Qualifications

• Technical School Diploma or LPN Program completed.
• Ability to establish and maintain control of violent patients or dangerous situations.
• Basic computer skills, critical thinking, and ability to concentrate.
• Knowledge and competence that reflects current nursing practice.
• American Heart Association Basic Life Support (BLS) - State of Pennsylvania Upon Hire

Responsibilities

• Assists provider by collecting patient history and reason for visit.
• Completes observation, physical exam, telephone triage, and analysis of diagnostic data.
• Ensures all laboratory and diagnostic tests are tracked and that the provider is presented with results.
• Provides education to patients and ensures education materials are current.
• Assists patients with prescription refills, appointment scheduling, and referral authorization.
• Assists providers during procedures and examinations and performs instrument and room clean-up.
• Coordinates internal and external appointments for patients based on medical needs and schedule.
• Intervenes to prevent or correct patient risks based on observations.
• Participates in telephone triage following protocols and provider orders
