Decreases fragmentation of care and coordinates services. Serves as a clinical liaison for the new and existing patients. Assists in administrative functions and in the development and implementation of patient care protocols, clinical pathways, and in updating patient policies and procedures. Coordinates efforts for treatment and promotes quality improvement and educational efforts. **Job Duties*** Helps patients and their family members \"navigate\" their medical care by helping them find solutions to treatment-related problems.* Assesses and monitors the continuity of care and expedites and coordinates the delivery of services to facilitate a patient's progress through their care.* Collaborates with the patient's medical team for follow-up and monitors patients' progress.* Participates in the development, process implementation, and review of quality metrics.* Participates in the development and implementation of patient education materials.* Identifies appropriate indicators to monitor and track patients in conjunction with marketing, patient data advertising campaigns, and patient referral outcomes.* Compiles data, tracks outcomes, and makes recommendations for process improvement.* Assists in the identification and reporting of quality issues to improve efficiency.
